Note (1) The application should be in quadruplicate and should be accompanied by four copies of the order of the Appellate Tribunal. (2) (i) The appeal should be accompanied by a treasury receipt in support of having paid the fee calculated at the rate of two per cent of the disputed tax and penalty subject to a minimum of rupees one hundred only and a maximum of rupees two thousand only where the application is preferred by the assessee. The fees should be credited in a Government Treasury to the appropriate head of account:     (ii)  Cheques, Drafts, Hundies or other negotiable instruments will not be accepted. (3) The application should be written in English and should set forth concisely and under distinct heads the fresh facts, which were not before the Appellate Tribunal at the time of passing the original order. There should be no argument or narrative.
